Microvascular decompression, while the most invasive of all options, is also the most effective at achieving long term remission of symptoms. Steps

Position patient for posterior fossa access

Achieve appropriate head and body positioning to optimize exposure of the cerebellopontine angle while maintaining airway and cardiovascular access. Proper positioning is critical for safe surgical approach.

Create skin incision for surgical access

Make a precise incision following anatomical landmarks to expose the underlying bone for craniectomy. Incision placement determines exposure quality and cosmetic outcome.

Perform craniectomy and open dura mater

Remove bone and incise the dural membrane to access the posterior fossa and cerebellopontine angle. This step enables visualization of neurovascular structures.

Expose cerebellopontine angle anatomy

Carefully retract cerebellum and arachnoid tissue to visualize the trigeminal nerve and offending blood vessels. Gentle dissection minimizes tissue trauma.

Decompress trigeminal nerve with interposition

Separate the compressing blood vessel from the trigeminal nerve root and place interposition material to prevent recurrent compression. This is the core therapeutic step.

Close dura, bone, and skin layers

Perform watertight dural closure, secure bone flap or craniectomy site, and close skin in anatomical layers. Proper closure prevents CSF leakage and infection.
